Error: Saving Annotation Failed in the Hypothesis LMS App

Users in the Hypothesis LMS app may encounter a “Saving annotation failed” error when trying to post annotations in an assignment. The image below shows how this error might appear within a Hypothesis-enabled LMS assignment.

If you see the “Saving annotation failed” error message in the Hypothesis LMS app, it’s usually due to one of the following reasons:

1. Unstable or No Internet Connection

Hypothesis requires a stable internet connection to save your annotations. If you’re offline or your connection drops, the app won’t be able to save your work.

Solution: Make sure you’re connected to a stable network before trying to save annotations.

2. Inactivity in an LMS Assignment

If you leave a Hypothesis-enabled LMS assignment open for a long time without any activity (e.g., an hour or more), the LMS may disconnect your session. When that happens, Hypothesis can no longer verify your identity, and saving fails.

Solution:

  • Copy any unsaved annotations into a separate document.

  • Close and reopen the assignment.

  • Paste your annotations back in and try saving again.
